It's our policy at SPS not to collect, use or share personal health information without the patient's consent to, unless permitted by law. Personal health information includes and is not limited to the name, address, OHIP #, date of birth and any other information regarding a patient's health including and not limited to medical history, medical condition, and any care the patient receives.

Service Providers: Like many businesses, SPS Connect uses third party service provides to assist in administering and providing the website and our Services. Providers assist SPS Connect in data storage, website hosting, marketing, etc. Your personal data will not be passed by our service providers for use by third parties in any form whatsoever unless we have obtained your consent or are legally required to do so.

Legal Requests and Obligations: SPS Connect may disclose personal data when required by law, such as to respond to court orders, subpoenas, or other legal processes by authorities with jurisdiction. SPS Connect may disclose data to meet its legal obligations.

Update your information: If you have an account with SPS Connect, you can update or make changes to your personal data by logging into your profile.  

Turn off notifications: If you have an account with SPS Connect, you can update or make changes to your personal data by logging into your profile or by contacting your individual services provider directly.  

  1. Account deactivation  

If you wish to deactivate you account with SPS connect you can do so by the following steps. Unlink any family/dependents then unlinking the account from the pharmacy. After these steps an option will then be available to “Delete” the account. Once the account is deleted all the information will be removed from the SPS connect app. If you want to register again you will need to go through the enrollment process again.

SPS Connect may change certain parts to this Privacy Statement from time to time, so be sure to check this Privacy Statement regularly. Any changes to this Privacy Statement will be communicated promptly on this page. Please contact the website administrator with any problems, questions, or suggestions that you may have. The constant development of the internet requires occasional adjustments to our privacy statement; therefore, we retain the right to make changes when necessary.
